CollaborateMD values your privacy and allows you to manage how your personal information is handled. CollaborateMD’s privacy policy is directly incorporated into this Agreement by way of this reference.
Whenever you are asked to create an account in order to use or access the Site or Service, you are required to finish the registration steps by supplying the full and correct details asked for on the registration form. You will additionally be asked to select a username and password. You alone bear full responsibility for keeping your password confidential. At no point may you use another individual’s account, username, or password. You consent to alert CollaborateMD right away regarding any unauthorized use of your account, username, or password. CollaborateMD will not be held responsible for any loss you may suffer because another person used your password, whether or not you were aware of it. You grant CollaborateMD a license to make use of the materials you post to the Site or Service. When you post, download, display, perform, transmit, or otherwise share information or other content (“User Content”) to the Site or Service, you are giving CollaborateMD, together with its affiliates, officers, directors, employees, consultants, agents, and representatives, a license to use User Content in relation to running the Internet business of CollaborateMD and those parties, which includes, without limitation, the right to copy, distribute, transmit, publicly display, publicly perform, reproduce, edit, translate, and reformat User Content. CollaborateMD may offer certain software to you through the Site. Should you download software from the Site, that software, along with all files and images it contains or produces, plus any accompanying data (collectively, the “Software”), is considered licensed to you by CollaborateMD strictly for your personal, noncommercial, home use. CollaborateMD does not convey either the title or the intellectual property rights to the Software, and CollaborateMD keeps full and complete title to the Software as well as every intellectual property right in it. You may not sell, redistribute, or reproduce the Software, and you may not decompile, reverse-engineer, disassemble, or otherwise turn the Software into a human-perceivable form. Every trademark and logo belongs to CollaborateMD or its licensors, and you may not copy or use them in any way.

Compliance With Intellectual Property Laws
In accessing the Site or making use of the Service, you agree to follow the law and to honor the intellectual property rights of others. Your use of the Service and the Site is always governed by and subject to laws concerning copyright ownership and the use of intellectual property. You agree not to upload, download, display, perform, transmit, or otherwise share any information or content (collectively, the “Content”) in breach of any third party’s copyrights, trademarks, or other intellectual property or proprietary rights. CollaborateMD maintains certain legally required procedures concerning claims of copyright infringement taking place on the Site or with the Service. CollaborateMD has established a policy providing for the prompt suspension and/or termination of any Site or Service user found to have infringed the rights of CollaborateMD or of a third party, or otherwise breached any intellectual property laws or regulations. It is CollaborateMD’s policy to look into any claims of copyright infringement brought to its attention. CollaborateMD retains the right to end your use of the Service and/or the Site. In order for CollaborateMD to deliver a high quality experience for you and other users of the Site and the Service, you agree that CollaborateMD or its representatives may access your account and records on a case-by-case basis to look into complaints or claims of abuse, infringement of third party rights, or other unauthorized uses of the Site or the Service. CollaborateMD does not plan to reveal the existence or occurrence of such an investigation unless the law requires it, but CollaborateMD retains the right to end your account or your access to the Site immediately, with or without notice to you and without liability to you, if CollaborateMD believes you have breached any of the Terms of Use, provided CollaborateMD with false or misleading information, or interfered with others’ use of the Site or the Service.

CollaborateMD has no control over, and no liability for, any third party websites or materials. CollaborateMD works with a range of partners and affiliates whose Internet sites may be linked to the Site. You agree to indemnify CollaborateMD for certain of your acts and omissions. You agree to indemnify, defend, and hold harmless CollaborateMD, along with its affiliates, officers, directors, employees, consultants, agents, and representatives, from any and all third party claims, losses, liability, damages, and/or costs (including reasonable attorney fees and costs) arising from your access to or use of the Site, your breach of these Terms of Use, or your infringement, or infringement by any other user of your account, of any intellectual property or other right of any person or entity. These Terms of Use shall be interpreted in accordance with and governed by the laws of the United States and the State of California, without reference to their rules concerning conflicts of law. You hereby irrevocably consent to the exclusive jurisdiction of the state or federal courts in San Mateo County, California, USA in all disputes arising from or related to use of the Site or Service.
Should a court of competent jurisdiction, for any reason, find any term or condition in these Terms of Use unenforceable, all remaining terms and conditions will stay unaffected and in full force and effect. No waiver of any breach of any provision of these Terms of Use shall amount to a waiver of any prior, concurrent, or subsequent breach of the same or any other provision, and no waiver shall be effective unless made in writing and signed by an authorized representative of the waiving party.

CollaborateMD League (“us,” “we,” or “CollaborateMD”) is dedicated to honoring the privacy rights of its patients, customers, visitors, and other users of the CollaborateMD Website (the “Site”). We prepared this Website Privacy Policy (this “Policy”) to give you confidence as you visit and use the Site, and to show our dedication to fair information practices and to safeguarding privacy. This Policy applies only to the Site, and not to any other websites you may reach from the Site, each of which may follow data collection and use practices and policies that differ substantially from this Policy.
We operate a general audience site and do not aim any of our content specifically at children under 13 years of age.
